Changes of Menu, Equipment, Services, or Commissary


Some changes need our review and approval, and some do not. Contact your inspector at 360-728-2235 to discuss the changes you want to make.​


Changes of Ownership


Food Establishment Permits are not transferrable from one owner to the next.

 

If the establishment is still open and operating, and there are no changes to the menu and equipment, submit the following applications at the same time (Please note that the applications can be found below in the applications section):

  1. Complete a Change of Ownership Notification Application.

  2. Submit and pay for the Change of Ownership Notification Application. A higher fee is assessed for notifications submitted more than 30 days from the effective ownership change date. Fees are non-refundable.

  3. Complete a Food Service Establishment Permit Application.

  4. Submit and pay for a Food Service Establishment Permit Application. Fees are non-refundable.

 

If there are any changes to the menu, equipment, business operations, or commissary, you must submit a Plan Review Application. These changes must be approved before you can begin operating.
