Please download, complete, and save the application. Then click the "Submit Online" button and attach your saved pdf to your submittal.

*Incomplete or inaccurate online applications may be refunded , and are subject to a $25 processing fee per the Environmental Health Fee Schedule. https://www.kitsappublichealth.org/fees

Refunds will be issued by check to the applicant and may take up to 3 weeks to receive by mail. If you have questions about your application, please call us prior to submitting at 360-728-2235.
