Change of Ownership Notification

If the establishment did not close during the change of ownership, download the notification application, fill it out, and submit using the buttons below.



A Change of Ownership Notification is not applicable if the establishment was closed for any amount of time. If the establishment closed for any amount of time, a Plan Review application is required.


Note that in either case, a Food Service Establishment Permit Application is also required because permits are not transferable between owners. You can apply for a permit here: https://www.kitsappublichealth.org/ehdocs/fsepermit

*Incomplete or inaccurate online applications may be refunded , and are subject to a $25 processing fee per the Environmental Health Fee Schedule. https://www.kitsappublichealth.org/fees


Refunds will be issued by check to the applicant and may take up to 3 weeks to receive by mail. If you have questions about your application, please call us prior to submitting at 360-728-2235.
