Chronic Disease & Injury Prevention

Protecting Your Lifelong Health

We all want to be healthy and live long lives—enjoying our families, remaining independent, and pursuing our favorite activities as long as possible. The kind of lifestyle we live — from childhood through adulthood — affects how healthy and active we can be.

Our program works in the community to support healthy behaviors and reduce health risks through collaboration, education, and the promotion of resources.

The foundations of a healthy lifestyle

The good news is that chronic diseases and injuries are largely preventable. These four habits, when combined with regular preventive care (such as tests for high blood pressure or diabetes), are proven to help prevent chronic disease and promote lifelong health:

  • Getting enough physical activity

  • Eating better

  • Not using tobacco products

  • Limiting use of alcohol and other substances

Our role

Our program works in the community to support healthy behaviors and reduce health risks through collaboration, education, and promotion of resources.
